Documentation is a daily responsibility for case managers. In the acute setting, automated systems have overtaken the author's ability to tell the patient's story. The case manager has an ethical duty to be thorough, objective and accurate. Legal duties and ethical obligations cross paths in the documentation of the case management process and promoting successful outcomes. This interactive session will raise important issues and provide guidance for ethical documentation.
