Embark on a quick journey of the trials and tribulations of implementing a demonstration project and hear client stories illustrating program successes. Judith will share her experience with a Medicaid demonstration project that provided her ambulatory clinic an opportunity to review their ability to identify high-risk clients and provide more efficient and coordinated care between nurse case managers, behavioral health practitioners, social workers and physicians in managing high-risk clients. Leave with key strategies for risk stratification in an ambulatory population and an understanding of the COACH model for case management.
