This symposia will demonstrate how a successful partnership between a large hospital system and a remote Care Management solution organization can collaborate and successfully launch a re-engineered Patient Access and Utilization Management program.  The presenters will share their recent experience as they worked together over several months to implement a robust patient review program to determine and document the medical necessity of a patient’s status upon bed placement thereby protecting the patient’s rights and financial obligation. Learn how this cooperative relationship helped the hospital system quickly and expertly ramp up their processes and staffing to attain expected outcomes – including the development of a frontline, patient advocate approach to improving physician documentation of medical necessity and enhancing the physician/nurse reviewer/payer relationship and their communication.

At Onslow Memorial Hospital, caregivers are using advanced, patient-centered machine-learning technology to better predict the individuals at risk of a readmission. During this session, Director of Clinical Informatics Autumn Foy MSN, RN will discuss her organization’s journey into applied predictive analytics. She will explore the reasons driving predictive analytic adoption within Onslow Memorial Hospital and the current and projected returns. She will also discuss how predictions fit into case manager workflow and the keys to successful predictive analytic adoption. Attendees will gain an understanding of what to look for in a predictive solution, how to drive the integration of new and emerging solutions, and how to measure predictive analytic value across caregivers and patients.
