An Evidence-Based Path to Value-Based Care
Timeslot: Tuesday, October 11, 2022 - 2:45 PM to 3:45 PM
Session Type: Concurrent Session
As the healthcare industry continues its shift from fee-for-service reimbursement models to value-based payments, providers are taking on increased clinical and financial risk for patient populations. This trend is helping to put a focus on quality outcomes and affordability at the individual patient level. In order for value-based care to be successful the following must happen: (1) Increased collaboration between payers and providers; (2) More efficient and effective care decisions based on vetted scientific evidence; and (3) A consistent approach of managing care with the patient at the center. Key elements that will enable this transformation include evidence-based, chronic care management and supportive change management within healthcare organizations. Successfully leveraging these elements will help patient-facing teams provide quality management of chronic conditions to optimize population outcomes. In this session, speakers will discuss frameworks for success, best practices, and real-world examples.
