In today’s value-based health care environment, improving the quality and experience of care while reducing the total cost is an expected demand for providers in any health care organization. With higher overall utilization, the Multi-Visit Patient (MVP) presents a clear opportunity for Health Systems to meet the needs of this population and address health inequities. This presents an additional opportunity as an integrated health system to improve care as an enterprise.
Stony Brook University Hospital [SBUH] is a 624-bed academic tertiary care hospital with ~100,000 ED visits and 30,000 inpatient visits annually. Our journey began with an extensive review of our visit data to determine what is an “MVP” by utilization standards. We ultimately developed a Care Management model that links our ED care management team to our outpatient care management team. To monitor our efforts, we created dashboards to visualize key important indicators.
In this session, the speakers will present one health system’s journey to understanding and addressing the needs of the Emergency Department (ED) Multi-Visit Patient [MVP], outline key takeaways and lessons learned.
