Hospital case management has been around for 30 years and continues to evolve. While many hospitals still rely on the discharge planning – utilization review framework popular in the 1990s, the move to value-based care coordination is gaining momentum as the marketplace shifts to new payment models, new delivery systems, at risk ACO contracts and population health. Hospital executives are cautiously preparing for a value-based environment through innovative disruption of the facility's case management program. Despite the uncertainties of the political landscape, healthcare organizations across the county are diving into initiatives that will help them succeed under emerging models.
