What's Next? Rethinking the Team

There is a need to find simplicity in the chaos of health care delivery, to rethink the role of the case manager as an advocate to coordinate an interprofessional team to best provide the right care, at the right time, in the right setting, at the right cost, with the right outcomes for the patients and clients served. In this session, the presenter will describe how case managers can, with curiosity, perseverance, and good old-fashioned grit, use the acknowledgment of what we do not know to create a concept of team, based on interprofessional care – a partnership between a team of health providers and the patient in a participatory collaborative and coordinated approach to shared decision making around health and social issues.
