Targeted Discharge Planning: Addressing Health Inequities to Reduce Readmission

Racial and ethnic minorities with socioeconomic disadvantages are vulnerable to 30-day hospital readmissions, widening the health equity gap in the U.S. In this session, the presenter will share the outcome of a 16-week quality improvement project aimed at decreasing readmissions of vulnerable patient populations through targeted discharge planning. The role of case managers in recognizing health disparities and adopting the improved process for discharge planning demonstrates the relationship of improved self-activation to the prevention of readmissions.
