Rachele Misiti, LCSW-S, one of the VA Western States Consortium Leads that provides support to the ongoing deployment of CC&ICM across VISN 17,19, 20, 21, 22. Prior to this, she served for four years as Assistant Chief of Social Work at Central Texas VA Healthcare System. After starting her career in community care (mental health and homeless case management) she transitioned to Central Texas VA in the Transition and Care Management program as a case manager in 2007. After years of providing case management to Seriously Injured Combat Veterans, she was promoted to the Transition and Care Management Program Manager for Central Texas Veterans Health Care System in 2014. Ms. Misti’s team was the recipient of the 2015 Transition and Care Management Team of the Year awarded by VHA Care Management Social Work Service. Ms. Misiti provided oversight to the rapidly expanding Veteran to Integration to Academic Leadership (VITAL) in Central Texas growing the program from one university to a total of six academic partners. Ms. Misiti was appointed social work site lead for Care Coordination and Integrated Case Management in Central Texas as early adopters in 2016 and has continued to serve in this role to maintain ongoing implementation. In partnership with Ms. Dorothy Sanders in Nursing, Ms. Misiti co-developed the innovative RACETIME operational model which has served as the gold standard for implementing an integrated approach to care coordination. In 2021, Ms. Misiti was appointed as the national committee lead for the VA Health Professional Scholarship Program (HPSP) for Social Work in its inaugural year. Ms. Misiti holds a Bachelor of Social Work and Bachelor of Studio Art from Bloomsburg University (2001), Bloomsburg, PA and a Master of Science in Social Work from the University of Texas at Austin (2004). Ms. Misiti is a proud 2021 graduate of Leadership VA (LVA). Ms. Misiti is an avid Dressage horseback rider, angora goat breeder and fiber artist.
