Pediatric care managers serve as the superheroes behind the scenes, assembling multidisciplinary teams, overcoming barriers, and guiding families through the often-fragmented healthcare system.

This session will introduce the essential components of pediatric care management, using a real-world patient story to illustrate how a child’s care can be transformed from chaos to coordination. Attendees will explore key elements of pediatric care management, including early screening and assessment, care coordination, family engagement, and cultural sensitivity.

Through the lens of our superhero framework, we will also examine the challenges case managers face, from maintaining clear communication among multiple providers to ensuring smooth transitions of care as children grow. Special attention will be given to the transition to adulthood, where tools like the Transition Readiness Assessment Questionnaire (TRAQ) empower adolescents to become their own healthcare advocates.
