This interactive case-based session will describe a hospital's innovative patient assistance program. Supportive Surroundings, a donor-funded program, offers financial assistance for discharge planning and provides a fully accessible apartment for short-term housing following a life-changing illness or accident. Attendees will learn how Supportive Surroundings addresses discharge barriers, improves the patient experience, and promotes, diversity, equity, inclusion, and accessiblity. Learn about the impact of the program on throughput challenges, length of stay, and health-related social needs through inspiring case studies.
