Breanne Muchemore, LICSW, CCM is the Senior Director of Care Management at Spaulding Rehabilitation Hospital (IRF) and Spaulding Hospital Cambridge (LTAC) working there since 2013. She oversees inpatient and outpatient case management and social work departments at Spaulding Boston and Cambridge. In addition, Breanne manages Complex Case Management and developed an innovative donor-funded patient assistance program, Supportive Surroundings, providing financial aid and short-term temporary housing. Breanne is a Licensed Independent Clinical Social Worker and Certified Case Manager. She earned her Bachelor’s Degree from Baylor University and is a graduate of Boston College with her Master's in Social Work. In 2020, Breanne managed Case Management teams at Boston Hope, 500 bed field hospital for patients with COVID-19 and post-acute care needs. Breanne is a Board Member for American Case Management Association-Massachusetts Chapter and the Co-Chair of the Education Committee.
